Once approved for enrollment into the Bethany Lutheran Preschool Program, the required forms below must be completed and submitted to the preschool office prior to your child’s attendance at school.  All forms submitted must be originals with signatures, no copies, pictures, or faxes.  Immunization Requirements

All children entering preschool must have received all the state-required immunizations.  We adhere to the “NO SHOTS, NO PRESCHOOL” policy.  A copy of your child's immunizations must be provided to the preschool.  Failure to comply with this regulation will cause your child to be excluded from preschool.   No child may enter preschool without all shots being up to date.
